5th Grade Immunization Clinic 9:00 - 4:00

Immunization Clinic for 5th grade students who will need the 6th grade TdaP booster before beginning the 6th grade. Parents need to send their original immunization card to have documentation of the shot added to the record. Northeast Colorado Health Department will be giving the shots. They do accept Insurance, Medicaid and CHP+. Please send a copy of the insurance card if you want that to be billed.
